The Southern New England Practice Transformation Network (SNE-PTN), a collaborative led by UMass Medical School and UConn Health, has been selected by the Centers for Medicare and Medicaid Services (CMS) to present its cost savings achievements at the 2018 CMS Quality Conference at the Baltimore Marriott Waterfront in Maryland Feb. 12 – 14.
SNE-PTN will be one of two networks from the Transforming Clinical Practice Initiative (TCPi) to deliver presentations on cost savings at the conference. SNE-PTN expects to achieve $59 million in cost savings over a four-year period ending in 2019.
The SNE-PTN team will detail the cost savings it has recognized as a result of working with a variety of clinicians, including optometrists, primary care providers and specialists. The costs savings are derived from four focus areas: emergency department avoidance for urgent eye care, improving HbA1c control among patient with diabetes, reduction in imaging studies in patients with low back pain, and eliminating clinically unnecessary anti-coagulation testing.
SNE-PTN is one of 29 practice transformation networks across the country chosen by CMS to be part of TCPi. SNE-PTN’s network partners include Baycare Health Partners, eHealthConnecticut, Healthcentric Advisors, Massachusetts Health Policy Commission, Qualidigm, Qualis Health and SMC partners, LLC.
